[Libs-Or] ILL labels

Hello:
We would like to change the way we identify books for ILL users.  We have seen many libraries using an actual label on the front of the book they lend us.  It appears to be printed out with Worldshare type information on it.

Two questions:
What label stock are you using?
What are you using as an information template for the label?
