[Libs-Or] account management

Hello -
How are libraries handling keeping track of all the various accounts (including passwords) that are created -  everything from social media to vendors?  Every time someone retires or leaves, we seem to miss a few and it turns into a major ordeal to get them updated.  I'm looking for some kind of centralized system or is Excel spreadsheet still my best bet?
