[Libs-Or] signed waivers

Just curious as to what other libraries do:  if you hold multiple programs during the year that require a signed waiver from parents, do you make the parents sign a waiver for each event? Do you, or would you, accept one waiver, good for the whole year?  If so, what is the wording you use?
