The State Library and OLA's Children's Service Division need to know if you want a free 2018 Collaborative Summer Library Program manual or not. Please take this survey if you do not want a free summer reading manual so we know your library didn't just forget to take the survey.

Remember to print the survey before you select "Done," but after you've answered the questions so you have a record of this information.
