[R2R-OR] 2011-2012 R2R Final Report forms on their way: due 12/1

Dear Ready to Read key contacts and library directors,

The 2011-2012 Ready to Read Grant final report forms went out in the mail this afternoon.  Library directors will receive the official copy of the final report form and key contacts will receive a working copy. As always, Ready to Read Grant final report forms are due on December 1st. Electronic copies of the final report form are now online at: http://www.oregon.gov/osl/LD/Pages/youthsvcs/aboutready.aspx#Forms

The final report form does not require an original signature which means you may submit them to Ferol Weyand via

*         Mail: Oregon State Library, 250 Winter St. NE, Salem, OR 97301

Something new:
Ferol will email library directors to confirm we received your final report form. The subject of her email will be "R2R Final Report Received (EOM)". EOM means End Of Message and you don't need to open the email because there will be no more message inside... the entire message is in the subject line! If you submit your final report and don't receive email confirmation from Ferol after 3-4 days, please contact us.
