[Self-Management_OR] Chronic Pain information

Dear colleagues,

We were fortunate enough to receive start-up funds to begin offering the Chronic Pain Self-Management Program this year in Central Oregon.  Understandably, our local grantor wants to ensure that we are able to sustain the program after the initial year-long pilot is over.

I'd like to get feedback from those out there in the self-management world who may be offering or thinking about offering the Chronic Pain Self-Management Program (CPSMP).  Could you let me know the following:


  1.  How long have you been offering the CPSMP?  How many leaders do you have?  How many classes have you offered?  Do you have a referral system set up with providers?
  2.  Do you collect pre and post data? If so, what instrument do you use?
  3.  What funding sources cover the expenses for the CPSMP?

I will plan to share details if others are interested, thank you in advance.
