[Self-Management_OR] Annual Quality Conference-November 15th!

Hello Oregon Self-Management Community,
Please join me, your local peers, and experts for HealthInsight's Annual Quality Conference, "Be the Change: Strategies for Health Care Transformation" in Keizer, November 15, 2016, at the Keizer Community Center. We have several sessions this year that I think are particularly relevant to self-management program providers. This is a face-to-face, interactive community event where we will explore strategies for transforming health care and engaging patients in an ever-changing environment.
We're excited to welcome keynote speaker Tiffany Christensen, a national patient and family engagement specialist from the North Carolina Quality Center. She will be joined by local thought leaders to share strategies that include:

*         Engaging patients and families in their care

*         Coordinating community resources to improve care transitions

*         Taking steps to help patients facing behavioral health issues

*         Adapting to the changing health care environment

*         Reducing burnout in health care

*         Using clinical pharmacy approaches for safer opioid therapy management

*         Applying concepts from the Total Cost of Care national project
