[Self-Management_OR] CDSMP (Living Well) Group Leaders Needed in SE Portland

Hello,

I am emailing again to see if there are any trained and active CDSMP group leaders who are interested in leading a "Living Well with Chronic Conditions" group at our clinic in SE Portland (located off of Powell and 90th).

The class is scheduled to begin January 8, 2018 at 5pm and continue for 6 weeks (Monday evenings).  Please email me back at gizellep at yvfwc.org<mailto:gizellep at yvfwc.org> if you are interested so that we may begin the process of getting you on our payroll before the class starts.
