Please consider applying to attend this four session Learning Collaborative. The deadline was extended until tomorrow.  This is invaluable to organizations serving Asian American people with Diabetes and Prediabetes.  Two Tuesdays in April and two Tuesdays in May.

AAPCHO, Joslin Diabetes Centers' Asian American Diabetes Initiative, Emory University, and the University of California, San Francisco School of Medicine is hosting a learning collaborative to share in-depth knowledge and effective strategies to support Asian American patients at health centers through non-clinical approaches.

Application for Participation
Asian American (AA) populations face unique challenges with respect to Type 2 Diabetes. While AA populations appear to have one of the lowest rates of diabetes compared to other racial/ethnic groups, disaggregated data show that specific subgroups have significantly higher rates of diabetes.

What is behind this disparity and what can health centers and other organizations do to support AA patients with pre-diabetes and diabetes?
Join us for a Learning Collaborative (LC) to learn more about diabetes and pre-diabetes disparities among the Asian American community. Guest speakers from Joslin Diabetes Center's Asian American Diabetes Initiative (AADI)<https://www.joslin.org/patient-care/multicultural-programs/asian-american-diabetes-initiative>, Emory University<https://diabetes.emory.edu/people/index.html>, and University of California, San Francisco School of Medicine<https://medschool.ucsf.edu/>, and University of California, Santa Cruz<https://www.ucsc.edu/index.html> will share in-depth knowledge and effective strategies to support Asian American patients at health centers through non-clinical approaches. Participants will engage in peer learning to share challenges and promising practices.

  *   Tuesday May 2, 7-8:30am HT | 10-11:30am PT | 1-2:30pm ET
  *   Tuesday, May 9, 7-8:30am HT | 10-11:30am PT | 1-2:30pm ET

Learning Objectives:

  1.  Understand the factors that influence diabetes disparities for AA patients.
  2.  Identify promising practices to implement effective diabetes prevention strategies for AA patients.
  3.  Engage in peer learning with participants on strategies and best practices to support AA patients with pre-diabetes and diabetes.

Intended Participants: Health centers, Primary Care Associations (PCAs), Health Center Controlled Networks (HCCN), and community organizations interested and/or invested in diabetes prevention and management for AAs.

Selection Criteria: Health centers and health center look-alikes will be prioritized for selection.

Fee: No cost/free

Commitment: Accepted LC applicants are asked to attend a total of four learning sessions between April and May 2023.
Application Deadline: Friday, April 7, 2023 by 5pm local time
