Good afternoon,

You are receiving this notification because your email address is associated with an active user account in the Oregon Compass self-management data portal. If you are no longer using your account, or feel you have received this communication in error, please don't hesitate to reach out so we can update our records.

We are writing to inform you that the following forms in Compass have been updated to reflect the latest changes made by the Administration for Community Living (ACL) and Office of Management and Budget (OMB):

  *   SMRC Program Participant Intake Survey (English)

*         SMRC Post-Program Survey (English)
*         WWE Participant Intake Survey (English)
*         WWE Post-Program Survey (English)

Please note this currently only applies to the English forms in Compass; Spanish forms are being updated now and should be available by early October. In the meantime, you can continue using the existing forms in Compass.

The updated forms will automatically be available in Compass for any SMRC and WWE workshops starting on 9/1/2023 and later. Workshops starting prior to this date will continue to utilize the old version of the form.
