<b>October 4</b>. SWARM created a form for agencies to use to submit the quarterly and annual performance measurement data. The form will be added to the OAM website once the policy is finalized.<o:p></o:p></span></p>

that tracks debt collection activities. The first report is due to DAS-SWARM <b>
no later than October 31</b>. The quarterly reporting measurements are documented in
<a href="http://www.oregon.gov/das/Financial/Acctng/Documents/35.60.20%20Performance%20Metrics.pdf">
ARPM policy 35.60.20</a>. Executive branch agencies, excluding elected officials, are required to submit the quarterly measurement data to
