[ASIST_Network] Change to DPSST CEU form for first responders

Dear ASIST Trainers,
The course number on the CEU form for law enforcement and other first responders to receive CEUs for ASIST (DPSST form) changed for 2012. The new course number is 12-0686.

The revised form is attached. Let me know if you need the instruction form.
