Dear Oregon ASIST Trainers,
We have been approved to offer 12.5 CEUs for ASIST trainings through Oregon NASW from July 18, 2013 through July 17, 2014. The CEs are applicable to social workers, counselors, and alcohol and drug treatment counselors and prevention specialists (and any others who accept Oregon NASW credits).
The documents are attached:
1. Roster - have participants sign it before the training; send to me after the training; they only get credits if they complete the full 2 day training.
2. Evaluation form - participants need to complete; send it to me after the training, with the roster.
3. Certificates of Participation - complete and give to each participant at the end of the training.
4. List of trainers who have been approved by Oregon NASW to offer the CEUs.
Please toss your old forms and replace them with the attached forms.
I have been out on medical leave until this week, so if you held a training between July 18th and now, the conference number needs to be changed on all the old forms. We can offer .25 fewer credits than last year so. Please inform participants if they have a certificate with the old conference number and no. of CEUs and ask them to correct them, or send them a new certificate.
if you aren't familiar with these forms, the Oregon Public Health Division is named as the training sponsor because the CEUs were approved through our completing the approval process for Oregon ASIST trainings.
If you (or your co-trainer) aren't on the approved trainer list and want to be, please send me a brief bio (1 paragraph with your education, employment, qualifications.) You are eligible if you are an ASIST trainer through LivingWorks.
