[ASIST_Network] CEUs for First Responders - 2015 form

Dear Oregon ASIST Trainers,
Happy New Year!

I've attached the 2015 form for you to offer CEUs to law enforcement and first responders for ASIST through the Oregon Department of Public Safety Standards and Training (DPSST) and the instruction form that goes with it. ASIST trainers need to fill out training details on the form (dates, sponsoring agency, location, instructor information, etc.  and send the completed form to DPSST (address at the bottom of the form). Participants will know their own DPSST numbers.

The documents for offering CEUs through Oregon NASW are good through July.
