Donna From: ASIST_Network [mailto:asist_network-bounces at listsmart.osl.state.or.us] On Behalf Of NOONAN Donna Sent: Thursday, June 23, 2016 2:47 PM To: 'asist_network at listsmart.osl.state.or.us' Subject: [ASIST_Network] Extended deadline for ASIST T4T applications to July 1 Importance: High Dear Oregon ASIST Trainers, We've extended the deadline to apply for the ASIST Training for Trainers (T4T) to the end of the day, next Friday, July 1! We are accepting applications to attend an ASIST Training for Trainers (T4T) to become an ASIST trainer through Friday, July 1st. The Living Works website says the training is closed because we have reserved the spots for successful applicants. There is still time and space to apply! Please feel free to contact me with any questions (donna.noonan at state.or.us). Good luck! We are offering scholarships for 12 Oregonians to attend the ASIST Training for Trainers on August 22 through August 26 in Medford, Oregon and become ASIST trainers! See below and the attached documents for application information. * What: The Oregon Public Health Division will pay the registration fees ($2885 per person) for up to 12 people to attend the Applied Suicide Intervention Skills Training (ASIST) Training of Trainers (T4T). ASIST is "the world's leading suicide intervention skills workshop." ASIST comes from LivingWorks Education LP in Canada. * Who may apply: Interested Oregonians who complete and submit the Trainer Candidate Application Form (attached), who have supervisor's written support (if applicable) and are able to meet the expectations described in the Trainer Agreement (sample attached). * Where: Medford, Oregon * When: August 22 through August 26, 2016 * How to apply: * Read and have your supervisor read (if applicable) Essential Information for Candidates and other documents here. * Get your supervisor's support and approval (if applicable). * Complete the Trainer Candidate Application Form (attached). * Send the Trainer Candidate Application Form to me (donna.noonan at state.or.us) by July 1st. We will consider all applications, giving special consideration to the potential impact on targeted youth populations, trainers taking a systems approach within their communities, and to geographic region. I will let you know whether you have been accepted or not early in July. If accepted, I will send you a trainer agreement to sign and return to me. At that time, we'll register you for the T4T. The 2-day ASIST training is listed and eligible for DPSST Course Hours (Course No. 16-0686). ASIST is on the Suicide Prevention Resource Center's (SPRC) Best Practice Registry. Find that listing here. It is listed as an evidence-based approved-practice by the Health Systems Division (formerly Addictions and Mental Health Division) in Oregon. It also is eligible for CEUs through the Oregon chapter of NASW. This ASIST Training for Trainers is funded through a Garrett Lee Smith (GLS) Youth Suicide Prevention grant to the Public Health Division from the Substance Abuse and Mental Health Services Administration (SAMHSA). Funding for this training is made possible by grant number SM 061759 from SAMHSA. The views expressed in written training materials or publication and by trainers do not necessarily reflect the views, opinions, or policies of CMHS, SAMHSA, or HHS; nor does mention of trade names, commercial practices, or organizations imply endorsement by the U.S. Government. The new conference number is 19-16-143. Please use the new forms (attached) and delete your old ones. Annual reminder: the documents you will need when you grant the CEUs to ASIST participants: 1. Roster form - every participant who wants NASW CEUs needs to sign - and print their name on - this form. Send it to me after the training. 2. Evaluation form - every participant who wants NASW CEUs needs to complete an evaluation form. Send them to me after the training. (It seems to work best to give them their certificates at the same time they hand in this form to be sure to get all of them.) 3. Certificate of Completion for Oregon NASW CEUs. Fill this out and give to participants. This is how they keep track of their own CEUs. PLEASE NOTE that the forms list the Oregon Public Health Division as the "conference sponsor." Leave this wording as is - it's there because we (Health Division) applied for and obtained approval for Oregon ASIST training participants to get these credits. In other words, we sponsor the CEU process for Oregon ASIST trainings. The new training "Conference Number," 19-16-143, is valid for these trainings, by Oregon NASW approved trainers, for a year (thru mid-July 2017). The information on where to send the completed rosters and evaluation forms is on the bottom of the forms. You may advertise that you offer NASW CEUs in your promotional materials. These credits also are accepted for LPC's and other professional counselors and for alcohol and drug prevention specialists and treatment counselors through ACCBO (the Addiction Counselor Certification Board). The most current list of approved trainers that I have is attached. If you have a training partner who is not listed, they need to send me a brief bio for me to send Oregon NASW for their approval. Please contact me with any questions. Thank you for the wonderful work you are doing by holding ASIST trainings! With Warm Regards, Donna Donna G. Please click the link below to tell me what date(s) you are available for a 1 day ASIST refresher. The coaching trainers from last year's ASIST refresher that was hosted by Columbia Care in Gresham and the LivingWorks ASIST conference grant are willing to help everyone refresh their skills as long as they are available! Please fill out the doodle poll and I will get a date for the training set sometime next week. I will then follow-up with some form of registration and ask how people want to tackle lunch - on our own or everyone bring $ and we'll order in, unfortunately my project cannot cover the cost of food. Please click the link below and see you at the refresher training! http://doodle.com/poll/xwpdhemt3swszgq5 Share this link with all those who should cast their votes.
