This payment is based reconciling the estimated payments made in May 2011 with the payments based on actual costs as reported in the High Cost Disability collection. Please review this spreadsheet. Unless changes are requested, the May 2012 reconciliation
for the High Cost Disability Grant will be made on this information.</div>
