<a href="http://www.oregon.gov/ode/students-and-family/fosteringconnections/Documents/fctransportgrantreimbursement.xlsx">
Invoice template</a> to report your quarterly transportation expenses for reimbursement.

<p class="MsoNormal">The due date for the <b>first quarter</b> reporting (7/1/17 – 9/30/17) has been extended to
<b>March 9, 2018</b>. Please note: Claim request for first quarter received beyond this date will not be processed.
