[Cnaprogram] New ADA Accommodation Request Form for CNA and CMA Certification Exam & FAQs

Hello,

There is a new form to request testing accommodation for the certified nursing assistant (CNA) or certified medication aide (CMA) state certification examination.  You can find the new Oregon State Board of Nursing Request for Testing Accommodations form at http://www.oregon.gov/OSBN/pdfs/form/LIC-614.pdf .  This form is available for use at this time.  However, the Board will continue to accept requests on the previous form until August 1, 2016.  After August 1, 2016, test candidates will be required to submit the new form for all requests for testing accommodations.
