[Cnaprogram] Reminder to always to use the most current application from the Board's website

I want to remind you that rules and policies change, and thus application forms change.  Always download the current application from the Board's website.

The current Oregon Application for CNA by Examination has a date of 02/01/18.  Please make sure that this is the version of the application that you are submitting.

Any applications received with an earlier published date will be returned with a request to submit the current application.
