http://www.oregon.gov/OSBN/pdfs/form/LIC-703B.pdf</a> when applying to retake the CNA or CMA exam. Do not fill out another initial exam application (form LIC-703 or LIC-704). Submitting another initial application slows the processing of the retake application.

<p class="MsoNormal">You may also just call the Board’s main phone number, 971-673-0685, and pay the retake fee to expedite getting released for a new exam date. Calling a licensing technician directly takes that individual away from processing exam applications
