all applicants (per Oregon Revised Statute 25.785) according to this schedule:<o:p></o:p></span></p>

<b>new</b> license or certificate: beginning January 1, 2018<o:p></o:p></li><li class="MsoNormal" style="color:#44546A;mso-list:l0 level1 lfo1">Applicants seeking to
<b>renew</b> their license or certificate: beginning April 1, 2018. <o:p></o:p></li></ul>

<p class="MsoNormal"><span style="color:#44546A">All SSNs will be disclosed to the entities and used for the purposes listed in Oregon Administrative Rule 851-001-0030 (2). Refusal to provide a SSN will result in denial of licensure/certification. This denial
will be reported to the National Practitioner Databank, as authorized by 42USC Section 666(a) (13). Applicants who are currently working on a US Visa (H1B, I-766 or other current federal government form authorizing them to work in the United States), may
submit copies of their passport and the Visa along with their application. Applicants who are attending school on an F1 Visa must provide a copy of the I-94 and I-20 signed by the designated school authority.”<o:p></o:p></span></p>
