[PacificSource Health Plans | Washington ...]2025 Community Capacity-Building Funding for HRSN services
The Oregon Health Authority (OHA) will be distributing Community Capacity Building Funding (CCBF) to coordinated care organizations (CCOs) interested in providing Health-Related Social Needs (HRSN) services such as medically necessary climate-related home remediations, housing-related supports, and nutrition-related supports.

How to apply: Applications for PacificSource CCBF funding will be accepted from April 1, 2025, to May 30, 2025.
To apply for CCBF, you’ll need to complete the CCBF application (PDF)<https://www.oregon.gov/oha/HSD/Medicaid-Policy/Documents/2025-CCBF-Application.pdf> and budget request form (XLS)<https://www.oregon.gov/oha/HSD/Medicaid-Policy/Documents/2025%20CCBF%20Budget%20worksheet.xlsx>. [Trillium Community Health Plan ...]The New Drug Talk: Community Edition Virtual Screening
As part of our ongoing commitment to supporting communities with behavioral health resources, Trillium has partnered with Song for Charlie to expand their parent and caregiver fentanyl education programs in Oregon. These materials include “The New Drug Talk,” a program designed to help parents and caregivers educate young people about the risks associated with fentanyl, counterfeit pills, and today’s drug environment.

This National Fentanyl Awareness Day, please join Song for Charlie for a special virtual screening of "The New Drug Talk: Community Edition." This session, moderated by prevention expert Marcia Lee Taylor, includes a screening of a 27-minute film for parents and communities, "The New Drug Talk," followed by Q&A/panel with impacted family members, medical professionals, and prevention experts. Topics covered include the current reality of fentanyl and fake prescription pills, how to have effective conversations with teens and young adults, warning signs and actions to take, and where to find naloxone and how to use it.

Milkweed and Honey is a fiscally sponsored project under CORE. Milkweed & Honey was founded by Nessa Bauer who has years of experience working in the field as a peer, and as a full spectrum doula. Milkweed & Honey is co-housed alongside CORE’s existing services.

Milkweed & Honey’s offers:
Doula Support: Culturally responsive care for young adult’s reproductive health journeys (pregnancy, loss, termination, reunification, birth control, etc.)

Peer Case Management: Housing, benefits, education, resource and systems navigation rooted in meeting you where you are at.
How to Refer:
Milkweed and Honey and CORE are both under the same Services Request Form.
