[Erate] Changes to USAC webpage address

USAC has made some minor changes to their domain structure recently that may affect saved links to the site. The URL www.sl.universalservice.org<http://www.sl.universalservice.org> is no longer functioning. The Schools and Libraries page of the USAC website can be reached by either www.universalservice.org/sl<http://www.universalservice.org/sl> or www.usac.org/sl<http://www.usac.org/sl>. Please update your bookmarks if you have any trouble. :)
