Hello, fatality review colleagues.

We would appreciate your nominations for the Theresa M. Covington Award for Excellence in Fatality Review. Nominations are due by July 8, 2020. Please follow this link to submit a nomination: https://www.surveymonkey.com/r/2020Covington

This award acknowledges and celebrates the significant contribution Ms. Teri Covington has made to the field of child fatality review. Child Death Review (CDR) became a known and respected public health and injury prevention program due, in large part, to her national leadership and the interdisciplinary partnerships she helped to build. Teri spent more than two decades working in fatality review. First, she built the Michigan CDR and Fetal and Infant Mortality Review (FIMR) programs and managed them. Then, she became the founding director of the National Center for Fatality Review and Prevention from its inception in 2002 through 2017.  She continues to be a tireless advocate for the health and wellbeing of children. The National Center presents the award annually to a fatality review professional who embodies Teri's qualities of leadership, collaboration, and advocacy.

Eligibility criteria for nominations:

  *   Fatality review professional in working in CDR
  *   Experience and innovation in leading fatality review
  *   Experience and innovation in collaborating with partners
  *   Experience and innovation in advocacy for the health and wellbeing of children
