Oregon County Child Fatality Review Survey




REMINDER: We need your input!

Oregon County Child Fatality Review - Brief Survey for Team Members



If you have not yet completed this brief but important survey of all county child fatality review team members, please do so now. The purpose is to:



  *   Improve the state team's understanding regarding how each local team conducts its child fatality reviews.

  *   Uncover the shared strengths and common challenges faced by local teams.

  *   Identify the ways in which the state team can best support the local teams based on local teams' common and unique challenges and concerns.

  *   Determine short and long term action steps that teams can take to improve the child fatality review process and strengthen its impact in the prevention of child deaths.

You may also receive a survey link from your team lead or coordinator and only need to complete once. The survey takes less than 10 minutes. Please complete the survey by Wednesday, September 15th.



The state team is committed to being transparent about who will see answers to the surveys. The state team co-chairs and the few individuals supporting the efforts will see the data. Only de-identified (no names), aggregate data (summarized and not information from an individual) will be shared with the state and county child fatality review teams. This summarized information may be included in a written report.
