Survey: Child Fatality Prevention and Review Program

Hello,



My name is Carmen Mims and I'm a Prevention Coordinator specializing in infant safety and wellbeing with the Child Fatality Prevention and Review Program within the Oregon Department of Human Services' Child Welfare Division. I am reaching out to invite you to complete a brief survey about infant safe sleep education efforts in Oregon. Your insights and experiences are extremely valuable and will help us better understand current needs, improve outreach strategies, and strengthen support for families and caregivers in Oregon to reduce preventable infant death.



This survey applies to any organization who interfaces with families even if you do not directly educate families about safe infant sleep as it explores your knowledge of other resources and supports available in the community to meet this need. If your organization provides safe sleep education directly to families, please have someone complete the survey who is knowledgeable about the educational content and approach.



The survey should take approximately 5-15 minutes to complete depending on the depth of your answers. Your responses will guide planning and evaluation related to infant safe sleep education efforts for Oregonians.  We are hoping to receive answers by Friday, March 20.
