Dear SNAP Community Partner Meeting Invitees,

SNAP Policy is writing to inform you that the updated form 349d is now available online. As mentioned in the January meeting, individuals whose food was destroyed in a disaster/misfortune can now list the total value of the loss, rather than listing out food items and their prices. The form can be used....

  *   To request replacement of SNAP or TANF benefits stolen electronically (for SNAP the theft had to occur on or before December 20, 2024), or
  *   To request replacement of SNAP benefits after food purchased with SNAP benefits was destroyed in a misfortune (including individual or community-wide disasters).
