The Oregon PRAMS (Pregnancy Risk Assessment Monitoring System) Team is currently in the process of revising our survey questionnaire.  We are in need of mothers with children under the age of 12 months to help field test 7 new survey questions.  If you fall into this category, or know anyone who is, that might be willing to help us by answering the questions and providing feedback, we would greatly appreciate your help!

Please see attached a word document containing the questions that may be added to the PRAMS survey questionnaire in the future. I have also attached a word document with a few suggestions on the type of feedback you can provide. However, we value any thoughts you might have regarding the questions, so if you have other comments you would like to make, please do not hesitate to provide them. 

We are happy to receive feedback in any form, for example:
1.     You can respond by email
2.     You can contact me by phone to discuss your feedback
3.     You can arrange to meet with me to discuss your feedback in person
4.     You can print out the questions or the feedback sheet and write your feedback directly onto the paper, then send it to me using the intra-office mailing system

We would like to receive your feedback before December 28, 2010.
