<DIV align=center><B><FONT color=#0082bf size=4 face="Times New Roman">KAISER PERMANENTE NORTHWEST</FONT></B><BR><BR><B><FONT size=4 face="Times New Roman">REQUEST FOR PROPOSAL (RFP) ANNOUNCEMENT OPPORTUNITY FOR </FONT></B><BR><B><FONT size=4 face="Times New Roman">ORAL HEALTH INITIATIVE FOR </FONT></B><BR><B><FONT size=4 face="Times New Roman">PREVENTION, ADVOCACY, ACCESS TO CARE, AND EDUCATION </FONT></B><BR><FONT color=#2f2f2f size=5 face="Times New Roman"></FONT></DIV><FONT color=#2f2f2f size=4 face="Times New Roman">PORTLAND, OR - </FONT><FONT size=4 face="Times New Roman">The Kaiser Permanente Northwest Region (KPNW) is seeking proposals from qualified community-based organizations, grassroots advocacy groups, dental care providers, non-service networks, health centers, and federally qualified health centers that best meet the qualifications and geographical requirements of the KPNW Region. We invite proposals from groups who are beginning to be involved in policy advocacy work as well as those who have been doing this for some time and are willing and able to take their work to the </FONT><BR><FONT size=4 face="Times New Roman">next level. </FONT><BR><BR><FONT size=4 face="Times New Roman">KPNW has dedicated a minimum of $750,000 over a three-year period for this initiative and intends to award a portfolio of five to eight grants in the $30,000 to $50,000 range annually as a result of this competitive RFP. Grant size will depend on experience, need, level of community support, and a clear plan and ability to demonstrate outcomes. Projects maybe funded for one year or up to three years. Grant awards will not exceed $50,000 per year, up to a maximum of $150,000 over a three-year period. Multi-year requests will be considered for up to three years (ending in 2014). Subsequent years of continual funding will depend on adequate performance in previous years. </FONT><BR><BR><FONT size=4 face="Times New Roman">KPNW is mandated by the IRS to support our service areas in both Washington and Oregon. These counties include:</FONT><BR><B><FONT size=4 face="Times New Roman">Washington:</FONT></B><FONT size=4 face="Times New Roman"> Clark, Cowlitz, parts of Wahkiakum* and Skamania* counties.</FONT><BR><B><FONT size=4 face="Times New Roman">Oregon:</FONT></B><FONT size=4 face="Times New Roman"> Clackamas, Columbia, Marion, Multnomah, Polk, Washington, parts of </FONT><B><FONT face="Times New Roman">*</FONT></B><FONT size=4 face="Times New Roman">Columbia,*Hood River,* Linn,* and </FONT><B><FONT face="Times New Roman">*</FONT></B><FONT size=4 face="Times New Roman">Benton counties.</FONT><BR><BR><B><FONT size=4 face="Times New Roman">*Organizations may email community.benefit@kp.org to </FONT></B><B><I><FONT size=4 face="Times New Roman">verify</FONT></I></B><B><FONT size=4 face="Times New Roman"> that they located in a Kaiser Permanente service area.</FONT></B><BR><BR><FONT size=4 face="Times New Roman">KPNW Oral Health Initiative RFP will provide funding to assist regional safety net partners in creating and implementing strategies to support oral health initiatives that address the following four priority areas: </FONT><BR><FONT size=4 face="Times New Roman">1. Increase access to oral health care by removing barriers that exist in health services and/or prevention and educational initiatives. </FONT><BR><FONT size=4 face="Times New Roman">2. Policy improvements at the institutional and/or government levels that will improve oral health in the proposed area.</FONT><BR><FONT size=4 face="Times New Roman">3. Enhance oral health education and prevention messages and interventions targeting consumers, public health, and medical professionals (e.g., pediatricians, dentists, hygienists, health educators, parents, and guardians).</FONT><BR><FONT size=4 face="Times New Roman">4.
