Registration for the Living Well Forum is now available online! The Living Well Forum is geared towards leaders, Master Trainers, program coordinators, and partners involved with the Stanford Living Well with Chronic Conditions, Tomando Control de su Salud, and Positive Self-Management Programs.  We encourage you to invite your partners, leaders, and others in your local community to build interest in the program!
 
Included in this year's Forum are:
-A keynote presentation by Teresa Brady, PhD, Centers for Disease Control and Prevention
-An update on state health reform efforts
-Three series of breakout sessions tailored towards program coordinators, partners, and leaders, including a Tomando Control breakout session held in Spanish
-Living Well Outstanding Practice Award nominations
