This is a friendly reminder that registration for the sixth annual Living Well Forum will close on Monday, May 2.  Registration for the Living Well Forum is available online at http://public.health.oregon.gov/DiseasesConditions/ChronicDisease/LivingWell/Pages/2011LivingWellForum.aspx.  You may also access the registration Web site directly at http://beattygroup.com/livingwell.htm.
 
The Living Well Forum is geared towards leaders, Master Trainers, program coordinators, and partners involved with the Stanford Living Well with Chronic Conditions, Tomando Control de su Salud, and Positive Self-Management Programs. 
 
The agenda for the Living Well Forum includes:
-A keynote presentation by Teresa Brady, PhD, Centers for Disease Control and Prevention
-An update on state health reform efforts
-Breakout sessions tailored towards program coordinators, partners, and leaders, including a Tomando Control breakout session conducted in Spanish
-Living Well Outstanding Practice Award nominations
