[HealthyTribes] Community Cessation Resources Update

Hi all, 
 
Each year, Free & Clear (our Quit Line service provider) updates the Oregon database of community-based cessation resources.  Free & Clear sends the attached survey tool to all programs currently in the database for their updates; however, this process does not capture any new resources, or those that may not have made it on to the list in the first place.  
 
We are beginning the 2011 update this week and would like to include you all, and your community partners.  I am attaching the most current spreadsheet of community-based cessation programs (sortable by city and/or county).  
 
Please take a look at available programs in your areas.  If there are additional programs that you are aware of that are not listed here, please either a) complete the attached survey for that program, or b) send the survey on to the correct contact(s) for that program for them to complete.  
 
Updates can be sent to the contact on the survey document or directly to me.  We are hoping to have all updates in by Friday, April 15th.  
 
As always, participating in this process is completely optional.  I'm happy to answer any questions.  Thank you!
