Hi all,
As mentioned on today's Quit Line demonstration call webinar, the Quit Line does maintain a database of community cessation and chronic disease resources and uses this database to refer callers to other services. We update this database annually. I know many of you are already familiar with this system and have provided updates to the database in the past. Thank you for that!
I am attaching the most recent version of the community resource database for your reference. If you have additional resources to add to the list, or if there are programs listed that are no longer available, you can send me an email at any point and let me know what changes should be made.
Please note: all Quit Line callers with chronic conditions are automatically referred to Living Well programs, so we do not include individual Living Well programs in this database. Other community resources for mental health, addictions, and other health issues that are not on our list of chronic health conditions will not be included in the database at this time.
And finally, you can always see if Quit Line callers are being referred to community resources by looking at the Monthly Referrals to Tobacco Resources and Monthly Referrals to Chronic Conditions Resources reports. The August 2011 reports are available online at www.box.net/quitlinereports/ and I am happy to provide any earlier reports if you need them.
