[HealthyTribes] Sharing the 2012 HPCDP Healthy places, healthy people: A framework for Oregon

Good Morning,

We are pleased to share the updated HPCDP Healthy places, healthy people:  A framework for Oregon which was finalized in October (see attachment). The Framework is HPCDP's guidance document that describes the conditions that must be in place at the state and community level to reduce chronic diseases among Oregonians. Conditions include policies, comprehensive tobacco and obesity programs, collecting and using data, education messages and access to a health system that supports chronic disease prevention, early detection and self-management.  The Framework provides the basis for HPCDP's work with grantees, contractors and partners.

The major changes from the original 2008 framework include: references to sugary beverages, active transportation, land use, and drinking water.  The healthy community diagram is included to illustrate the interaction and relationship between multiple settings and organizations to achieve healthy communities and healthy Oregonians. The diagram was created for the Public Health & Primary Care: The Backbone of Community Health and Wellness brochure.

Grantees are encouraged to use and share the Framework and the Public Health & Primary Care brochure in their work and with partners.
