Dear Tribal Health Administrators -

It was great to see you at the SB 770 meeting earlier this week. Following is information regarding two items: Healthy Communities Implementation Program funding results, and Centers for Disease Prevention and Control (CDC) Community Transformation Grants technical assistance opportunities.

Healthy Communities Implementation Program funding results
The Health Promotion and Chronic Disease Prevention Section (HPCDP) has fulfilled providing notification to all proposers about the results of the competitive Healthy Communities Implementation funding opportunity, as it pertains to their individual proposal. In follow up to this notification, we can now distribute the ranked list of scored proposals submitted. Please see the attached document for more information. Funding for Healthy Communities Implementation programs come from various CDC chronic disease program grants. The attached document identifies those who will be awarded funding beginning July 1, 2012, those who are currently waitlisted pending notification of receipt of competitive CDC grant funds ("TBD by June 30, 2012"), and those proposers who did not score high enough on the list to secure funds, given the CDC resources currently available.

HPCDP is compiling reviewer feedback so that information can be shared with all proposers as soon as possible. HPCDP wishes to express our deep appreciation to all who submitted a proposal to this funding opportunity. We wish there were sufficient resources to support all local programs with these efforts, but unfortunately that is not the case. We hope that communities will be able to use the products developed through the Healthy Communities proposal process to identify other resources to support these important efforts.

CDC Community Transformation Grants technical assistance opportunities
On a related note, the CDC recently released a competitive funding opportunity<http://www.grants.gov/search/search.do;jsessionid=RgW1PFPFQshPqDGLW5sBW8ZQTmTpXQwjMLllqTy7zJdwfc1QLpXP%21-1021879135?oppId=173114&mode=VIEW> ("Community Transformation Grants - Small Communities") that is closely aligned with the goals of Oregon's Healthy Communities Implementation Program. It is our understanding that these grants intend to distribute resources directly into local communities (rather than a pass-through scenario) to support the implementation of evidence-based prevention strategies to prevent heart attack, stroke, diabetes, and other leading chronic disease-related causes of death and disability. Letters of Intent to apply are due to CDC June 18. Proposals are due to CDC July 31.

HPCDP wishes to provide technical assistance and coaching support to tribes in preparing competitive proposals to CDC in response to this opportunity. HPCDP will host a conference call for all interested tribes and county health departments to learn more about this funding opportunity, and to outline our plans for technical assistance support. This call is scheduled for Wednesday, June 13th from 1:30-2:30pm. The conference call information is below:

On the call, we will describe the technical assistance support we plan to provide, which will include template narrative documents, sample plan objectives, and an opportunity for having drafts reviewed for those submitted to HPCDP by July 16, 2012.
