Eligible applicants include local public health authorities, nonprofit social service providers, health care organizations, public health research organizations, and non-traditional partners such as businesses, economic development organizations, private funders, and faith-based organizations. Local public health authorities play a key role in community health and can lead or participate in regional coalitions. When not leading, local public health authorities are key partners and stakeholders, who may provide data and identify best practices, policy, system and environmental change to improve health.



OHA anticipates awarding no more than three (3) Grant Agreements of up to $130,000 each,

for the period of November 1, 2013 to October 31, 2014.



OHA will award grant(s) ("Grant(s)") to fund the establishment of up to three (3) RHECs which are coordinated, collaborative groups organized at a regional level to address issues that create inequities in health and social well-being, identify policy, system and environmental changes, and craft and implement strategies and policies to increase equity and address the social determinants of health through the transformation of Oregon health systems, coordinated care organizations and other policy avenues. The focus of the work of RHECs is to organize communities historically barred access from decision making tables.
