This is a request for you to assist in a Research Study "The Role of Cooperative Extension Service in Tobacco Control Coalitions".

Please forward these email to the chairs or paid staff for all tobacco control coalitions within your state - local, county, regional, and state-level coalitions.
This is a request for you to take part in a research project. The purpose of this survey is to assess the Cooperative Extension Service System's role in local/county level coalitions that address health issues, including substance abuse with specific interest in tobacco use
Your involvement in this project will be kept as confidential as legally possible. All data will be reported in the aggregate. The questionnaire will not ask any information that will reveal your identity as a participant. Your participation is completely voluntary and you must be 18 or older to participate in this research project. You may skip any question that you do not wish to answer and you may discontinue at any time. West Virginia University's Institutional Review Board's acknowledgement of this project is on file.
The purpose of this survey is to assess the Cooperative Extension Service System's role in your coalition. The Cooperative Extension System is a nationwide, non-credit educational network. Each U.S. state and territory has a state office at its land-grant university and a network of local or regional offices. These offices are staffed by one or more experts who provide useful, practical, and research-based information to agricultural producers, small business owners, youth, consumers, and others in rural areas and communities of all sizes.
Please answer all questions as they relate to or best describe your coalition, the coalition's efforts, and the coalition's interaction with the Cooperative Extension Service System. Remember responses should be from a coalition viewpoint and not from you as an individual or as from your individual agency/organization. Only one designated person in each coalition should respond to this survey.
