[HealthyTribes] Place Matters - seeking local leaders for panel

Hello HPCDP County and Tribal Grantees (please excuse any cross postings):

This email is to ask for your help to identify local leaders to be part of the Plenary Panel at the Place Matters Conference on Friday, November 21, 2014. We're looking for two or three leaders who have been part of a successful policy or environmental change at the local level.  As in past years, hearing from community leaders about local successes has been a great way to end the conference and inspire others to action.

Description of the Plenary Leader Panel: We have the power to change the places we live, learn, work, and play to put healthy options within reach. The panel will highlight initiatives and innovative collaborations that are creating supportive environments to help Oregonians live tobacco-free, eat well and move more. Panelists will include elected and appointed officials and business and community leaders. The leaders will share their experiences of engaging with others to adopt and implement policies that promote health.

Please consider this opportunity to acknowledge a local champion! Send the following information to me by Friday, August 8, 2014:

*         The specific policy or system change achieved
