Learn about innovative projects Wisconsin is piloting to increase health promotion programs in rural communities. They’ll share their work and results from a 16-county process improvement strategy that aims to help jumpstart rural communities and explain the results and lessons learned. They will also explain a referral initiative with rural communities based on the understanding that everyday medical providers serve individuals that could benefit from self-management education. Learn different referral models rural Wisconsin communities have piloted that increase participation in EBPs. Hear best practices, lessons learned, and ways you can work with smaller health care systems to reach more of your population.
