[HealthyTribes] New data available on the HPCDP data and reports website

Hello,

Web tables on the HPCDP Data and Reports website have been updated and new tables added. The web address is below:

Website: https://public.health.oregon.gov/DiseasesConditions/ChronicDisease/DataReports/Pages/index.aspx

Below is a summary of what was changed or added:


*         BRFSS adult and trends over time tables were updated with 2014 data

*         OHT state and county tables were updated with 2015 data

*         A new web page called "Self-management and healthcare data" was added

*         New Stanford self-management programs tables were added (in county and the new web page mentioned above)

*         New hospital discharges tables were added (in county and the new web page mentioned above)
