Hello,

I am writing to share an update about the Healthy Communities grant opportunity offered through the Health Promotion and Chronic Disease Prevention Program. HPCDP anticipated releasing a three-year, competitive RFP for Healthy Communities (Program Element 15 Counties and Program Element 14 Tribes) to begin in July, 2015. Due to funding constraints and evolving funding discussions between OHA and CLHO, HPCDP will delay the release of a competitive Healthy Communities RFP for at least a year. Instead, current Healthy Communities grantees will have the opportunity to request a one year extension of their current grant. Please see the attached document for more information about this change.

For those who are current Healthy Communities grantees, please connect your liaison and Sabrina Freewynn (Sabrina.l.freewynn at state.or.us<mailto:Sabrina.l.freewynn at state.or.us>) at your earliest convenience to let us know if you anticipate taking the opportunity to continue for an additional year. Please plan on an up to 10% reduction from your current funding level. HPCDP will work to minimize this funding cut. We anticipate releasing guidance for a continuation application in April.

We know this is an unexpected and challenging change, particularly for tribes and counties that were preparing to apply for a new funding opportunity.
