Background: A Public Health and Social Justice Issue
People living with chronic mental illnesses die 25 years younger than the general population. This striking disparity is due to health issues caused by tobacco addiction and poor nutrition. Those with mental illness or addictions disorders smoke at much higher rates than the general population - with some diagnoses reaching upwards of 90 percent. Tobacco industry targeting to those living with mental illness or addictions is well documented.
In 2009, consumer advocates, government leaders, health care providers and community partners joined together to address this considerable social justice issue. Implemented in 2013, the Tobacco Freedom Policy:
* Requires all licensed residential providers to maintain tobacco-free properties;
* Includes guidelines to help residents and staff quit;
* Has strategies to make stopping tobacco use part of mental health and addictions treatment planning; and
* Promotes insurance-based benefits to help employees stop using tobacco.
2014 Survey with Residential Treatment Facilities
In 2014, as part of the Cross Agency Health Improvement Project<http://public.health.oregon.gov/PreventionWellness/HealthyCommunities/CAHIP/Pages/index.aspx>, the OHA Health Systems and Public Health divisions partnered to conduct a survey with residential treatment facility administrators to: 1) collect feedback on the successes and lessons learned around implementing the policy; and 2) identify supports to help reinforce tobacco-free living with consumers and staff. The survey results demonstrate that the Tobacco Freedom Policy is successfully being implemented in the majority of residential treatment facilities.
Key findings from the survey:
* Tobacco use is not occurring on most properties.
* Significant progress has been made in practices to support consumers interested in quitting.
* Consumers are quitting at the same rate as the general population; and
* Providers recognize the policy helps create an environment that supports health.
