[HealthyTribes] Scheduling reporting interviews

Dear Healthy Communities Coordinators,
It is time for the third in this 2015-2016 grant year reporting interview on the Healthy Communities grant. We are looking forward to hearing about the excellent work you have been doing and the challenges you face, if any. We use the information gathered for tracking progress and also for making decisions on training content, resources, and state-wide strategy. Your voice contributes to our collective conversation.

The report will cover the period of February through May and projecting through the end of the grant year. The interviews should be conducted between June 8th and 30th, 2016. You can expect the interview to take approximately one hour.

After you schedule your interview, your liaison will send you the reporting tool, which will include milestones from your 2015-2016 work plan. Please feel free to use the tool to make notes or gather documents prior to the call. Your liaison will complete the form during the interview and send the summary to you for your review and edits.
