A couple of important notes are included below:

  *   Tribal ADPEP and TPEP have moved from HPCDP to the Health Systems Division, My'Kee Martinez is the point of contact: Michael.Henry.Martinez at dhsoha.state.or.us<mailto:Michael.Henry.Martinez at dhsoha.state.or.us>
  *   Period 2 Reporting: Links to complete the online reporting form have been sent via email. Reports should be submitted by August 15, 2022. If you have any questions please contact your Community Programs Liaison.
