Reporting Pilot </span></b><span style="color:black;text-decoration:none"><br>
Daniel Morris, HPCDP Evaluation Lead and staff from NPC Research have worked with CLHO Chronic Disease volunteers to propose a new method for gathering information quarterly from TPEP coordinators. Those who volunteered to pilot the reporting method have now
completed the report with their liaisons, using sample tools – a set of interview questions and a form to fill out the summary information from the call. Liaisons collected the information and provided a summary back to the coordinators. The pilot will now
be evaluated by NPC Research, through feedback with coordinators and liaisons.</span></a><o:p></o:p></p>

Well and Department of Corrections<br>
</span></b><span style="color:black;text-decoration:none">The Oregon Department of Corrections will be offering Living Well workshops at all of its prison facilities over the next few months. In preparation, a Living Well leader training took place in Salem
in October for DOC staff in Western Oregon, and a second leader training is scheduled for November 17, 18, 24 and 25 in Umatilla for DOC staff east of the Cascades. The first wave of this partnership will include initial workshops provided by DOC staff, who
will then identify inmates to be trained as peer leaders in the second wave of trainings. DOC intends to do an evaluation of the program based on Stanford’s original research and will link attendance to the inmate’s medical records in order to measure the
impact of Living Well on health care utilization.</span></a><o:p></o:p></p>

Self-Management Collaboration </span></b><span style="color:black;text-decoration:none"><br>
Recruitment is now finished for the second OPCA Self-Management Collaboration cohort; Multnomah County clinic will have representatives of 9 clinic sites participating, Community Health Center of Lincoln County/Newport and Yakima Valley Farm Workers Clinic’s
Lancaster Street (E Salem) and Salud clinics (Woodburn). Cohort 2 clinics attended two webinars in October prior to the November 14 in-person kickoff meeting. The first was an orientation to the collaborative that focused on the benefits of community/clinic
partnerships and the second covered establishment of collaborative aim statements (goals). Clinics and community partners will sign letters of understanding outlining their roles in the project.</span></a><o:p></o:p></p>
<p class="MsoNormal" style="margin-bottom:10.0pt;line-height:115%"><b>APHA Conference Presentations

HPCDP is presenting two posters at the American Public Health Association Conference this month. One poster gives an overview of our hookah initiative, including policy opportunities. The other poster shows our Tobacco Control Integration Project (TCIP) model
and provides successes to date. This update will include a description of each poster and gives grantees a visual of some of HPCDP's TPEP work.

TPEP and Healthy Communities listserves on October 20 and tribal coordinators October 21.
