<p class="MsoNormal">As you may know, the Oregon Health Authority is moving forward with the development of local Coordinated Care Organizations (CCOs), which will be responsible for providing integrated physical, mental, and eventually dental health care for
the state's 600,000 Medicaid members. CCOs will be local entities, tasked with convening a governing board and community advisory council. The community advisory council is responsible for overseeing the development of a community health assessment and community
health improvement plan. CCOs are encouraged to work with local partners, including public health, to address prevention, wellness, and health equity.<o:p></o:p></p>
<p class="MsoNormal"><o:p> </o:p></p>
<p class="MsoNormal">HPCDP Connection now has a few documents that may be of interest to you if your organization is or is considering working with your local CCOs. These resources include:<o:p></o:p></p>
