<p class="MsoNormal">This is for your information only; no action is required. HPCDP will be conducting a survey of organizations offering Diabetes Self-Management Education/Training (DSME/T) programs throughout Oregon that have been recognized by the American
Diabetes Association (ADA) or accredited by the American Association of Diabetes Educators (AADE). Information will be collected on the providing organization, location where programs are delivered, and program capacity. The survey will sent to DSME/T program
coordinators next week; a report with aggregate survey results and a statewide map of DSME/T programs is expected to be available by June 2013.
