<p class="Default"><span style="font-size:11.0pt;font-family:"Calibri","sans-serif";color:windowtext">The Health Promotion and Chronic Disease Prevention Program (HPCDP) of the Oregon Health Authority announces the release of the 2014-15 Tribal and County Tobacco
Prevention and Education Program (TPEP) Request for Applications and the Healthy Communities Implementation Program Continuation Application. These documents will guide Local Health Departments (LHDs) to write work plans which implement evidence-based best
practices for commercial tobacco prevention and education programs and for prevention, early detection, and self-management of chronic diseases. The documents are available through HPCDP Connection here: https://partners.health.oregon.gov/partners/HPCDPConnection/Pages/index.aspx
