Community-Based organizations that will participate in the three Sustainable Relationships for Community Health (SRCH) learning institutes.

<p class="MsoNormal"><span style="font-size:12.0pt;font-family:"Georgia","serif"">These institutes will consist of facilitated discussions and technical assistance to engage leaders across CCOs, LPHAs, and Community-Based organizations involved in health system
transformation. The SRCH Institutes will include local and national experts who will share content related to models of care for heart disease, diabetes and prediabetes.<o:p></o:p></span></p>
<p class="MsoNormal"><span style="font-size:12.0pt;font-family:"Georgia","serif""><o:p> </o:p></span></p>
<p class="MsoNormal"><span style="font-size:12.0pt;font-family:"Georgia","serif"">Institute participants will specifically address issues related to:<br>
• areas of quality improvement, including use of quality measures, electronic health records and health information technology, and traditional health workers in team</span><span style="font-size:12.0pt;font-family:"Cambria Math","serif"">$B!>(B</span><span style="font-size:12.0pt;font-family:"Georgia","serif"">based
care. <br>
• increasing the use of evidence-based Community Self-Management Programs (CSMPs) through health system referral and reimbursement. <br>
<br>
The focus of these institutes will be on diabetes, prediabetes, and hypertension management. <br>
<br>
SRCH participants will be able to:<br>
1. Delineate roles and responsibilities<br>
2. Identify staffing and training needs<br>
3. Outline data sharing and payment agreements<br>
4. Ultimately create mechanisms to facilitate better care, better health, and lower cost.<br>
<br>
The SRCH Institutes will support LPHAs, CCOs, and CSMP Providers in developing formal commitments, such as memoranda of understanding and data-sharing agreements, to reinforce collaboration and a long-term commitment to health system improvement and community-clinical
linkages.<o:p></o:p></span></p>
