<p class="MsoNormal">It is time for the second in this grant year reporting interview on the Healthy Communities grant. We are looking forward to hearing about the excellent work you have been doing and the challenges you face, if any. We use the information
gathered for tracking progress and also for making decisions on training content, resources, and state-wide strategy. Your voice contributes to our collective conversation.

<p class="MsoNormal">The report will cover the period of November 1, 2015 through February 15, 2016. The interviews should be conducted between February 15<sup>th</sup> and March 15, 2016. You can expect the interview to take approximately an hour and a half.<o:p></o:p></p>
<p class="MsoNormal"><o:p> </o:p></p>
<p class="MsoNormal">After you schedule your interview, your liaison will send you the reporting tool, which will include milestones from your 2015-2016 work plan and a few additional questions about the Place Matters Oregon website and FB page. Please feel
free to use the tool to make notes or gather documents prior to the call. Your liaison will complete the form during the interview and send the summary to you for your review and edits.
